Schedule:
Saturday/Sunday Responsibilities
Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:
- Conduct Start of Care (SOC) Admissions — OASIS
- Conduct follow‑ups, evaluation visits as needed in designated territory
- Make home visits to patients in designated territory
- Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
- Monitor clients’ conditions, reporting changes to Clinical team
- Collaborate with physicians and agency staff to deliver the best care possible
- Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided
- Concierge for Case Managers: dedicated resources assist with time‑consuming administrative tasks
- Flexible work schedules: create your own schedule and hours
- Targeted territories – patient visits typically within a 5–15 mile radius
- Autonomy: no clocking in/out

- Current and unrestricted California RN license
- Current BLS/CPR Certification
- Valid California Driver’s License
- Experience conducting assessments and Start of Care (SOC) — OASIS highly desired
- High level of clinical knowledge in home health care highly preferred
- Experience with high‑acuity patients a plus
- Experience working with an interdisciplinary team highly desired
- Case Management fundamentals highly desired
- Previous experience with IV medication infusions highly preferred
- Proficiency in initiating intravenous (IV) therapy a plus
