Part‑time Registered Nurse Case Manager

Please note: All opportunities at 21

HHS require being in the field visiting patients in their homes.

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:

• Conduct Start of Care (SOC) Admissions — OASIS
• Conduct follow‑ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided

• Concierge for Case Managers: dedicated resources assist with time‑consuming administrative tasks
• Flexible work schedules: create your own schedule and hours
• Targeted territories – patient visits typically within a 5–15 mile radius
• Autonomy: no clocking in/out

• Current and unrestricted California RN license
• Current BLS/CPR Certification
• Valid California Driver’s License
• Experience conducting assessments and Start of Care (SOC) — OASIS highly desired
• High level of clinical knowledge in home health care highly preferred
• Experience with high‑acuity patients a plus
• Experience working with an interdisciplinary team highly desired
• Case Management fundamentals highly desired
• Previous experience with IV medication infusions highly preferred
• Proficiency in initiating intravenous (IV) therapy a plus
