Job: Occupational Therapist (OT) Home Health - Full Time or Part Time

Location: San Mateo, CA

We are seeking an experienced, self‑driven Full‑Time / Part Time (Per Diem) Occupational Therapist (OT) to join our dedicated Home Health team and provide exceptional care throughout the region. As an OT, you will evaluate patients’ health, create and implement care plans, and track progress to maintain the highest standards of care, working closely with our Clinical Director and Clinical Staff, both remotely and independently in the field.

• Provide, coordinate and oversee skilled intermittent care to patients, including instruction and education of patients and/or caregivers to promote maximal functional status in the patient’s home.
• Regularly evaluate and re‑evaluate patient status.
• Administer treatments, monitor patient conditions, and report signs/symptoms indicating a change in patient condition to Physician, Case Manager, and/or Director of Nursing as needed.
• Initiate, develop, implement and revise the patient’s plan of care (POC).
• Address all problems identified in the POC.
• Provide educational information to the patient/family/caregiver regarding knowledge deficits or needs concerning the patient’s condition.
• Communicate with other disciplines involved in the case as warranted.
• Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
• Comply with all company policies and procedures to ensure high standards of care.

• Current OT License to practice in California – Required.
• Experience in Home Health or Hospice care – 1 Year Required.
• Current CA Driver’s License.
• Able to travel within a 15–25 mile radius on a regular basis.
