Life Moves seeks a dedicated Case Manager to provide intensive, field-based services to unsheltered individuals, managing a caseload of 15–20 clients and developing housing plans based on Housing First principles.
You will coordinate with MDTs, agencies, and shelters, engage clients in trauma-informed care, and assist with housing applications and referrals. Evening/weekend availability may be required. Bilingual Spanish is a plus.
