Starting Salary Range:
Bachelor's Degree: $59,826 - $71,791;
Master's Degree: $65,929 - $79,114

This role offers a hybrid schedule, allowing you to work within clients’ homes in San Mateo County, our San Mateo office, and remotely from home.

• Participate in the assessment of individuals and their families, developing, implementing, and coordinating Individual Program Plans.
• Manage a wide range of active cases requiring professional social‑worker skills.
• Conduct ongoing social assessments, gather data for interdisciplinary teams, and monitor program outcomes.
• Initiate and terminate regional center funding, ensure purchased services are delivered, and prepare required reports and documentation.
• Advocate for individuals and families to secure entitled services and encourage participation in community resources.
• Initiate referrals, monitor vendor services, evaluate effectiveness, and identify community service gaps.
• Provide outreach services as assigned and participate in internal and community committees and task forces.

• Knowledge of counseling principles and techniques, individual and group behavior, human growth and development, family relationships, and community organization.
• Ability to work independently, exercise judgment, collaborate with an interdisciplinary team, and communicate professionally.
• Excellent written and verbal communication skills.
• Fluency in Spanish, with proficiency in spoken and written Spanish, including relevant dialects.

• Bachelor’s Degree in social welfare or a related human services field (Master’s Degree preferred).
• Case‑management experience strongly desired.
