In this leadership role, reporting to the Director of Nursing, you will be responsible for ensuring that patient care is coordinated and managed appropriately by a team of Registered Nurse (RN) Case Managers and LVNs. You will serve as the first point of escalation, ensuring that support is easily accessible in a timely fashion. You will also participate in identifying, developing, designing, and implementing nursing educational programs to support clinical excellence, determine success measures, and evaluate the effectiveness of these programs to promote ongoing staff development and patient and customer satisfaction.
You will meet regularly with our customers to maintain relationships and provide updates such as readmission reports, offering insights to improve patient outcomes and reduce readmissions.
This is a hybrid of field work and office‑based duties at the 21
HHS office:
- Office: Spend 2 days per week in the office for administrative and team management duties.
- Fieldwork: Spend 3 days per week (average 12+ visits per week) in the field conducting supervisory visits and offering direct support to RN Case Managers.
- In the field, oversee clinicians who provide care and treatment to patients in their place of residence.
- Handle incoming calls timely, and follow up as necessary to ensure appropriate actions are taken to meet internal and external customer needs.
- Coordinate training and educational programs for nurses within the field to improve clinical outcomes and competencies.
- Assess staff learning needs and formulate specific plans and goals for the next evaluation period with employees, perform administrative call duties, and serve as a role model and resource for staff.
- Act as the first point of contact for field clinicians to assist in patient care escalations.
- Coordinate at least bi‑weekly case conferences with the clinical team to discuss patients’ progress and revise the care plan as needed to meet planned outcomes.
- Review and evaluate each case by reviewing the services provided by clinicians, conferences, record review, discuss and verify impressions, instruct and guide clinicians to promote more effective performance and delivery of quality home‑care services, and remain available at all times during operating hours to assist clinicians as appropriate.
- Review patients’ clinical diagnoses, medications, procedures and clinical course.

- Competitive compensation
- Two years’ management or supervisory experience within the past five years in a health‑care setting, preferably in home health.
- Graduate of an accredited school of nursing. Bachelor’s degree preferred.
- Current CA Registered Nurse license (RN).
- Current Basic Life Support (BLS) certification.
- Valid CA driver’s license with an acceptable driving record. All license and certifications must be current at the time of hire and sustained throughout employment.
Salary: $155,000 – $170,000 per year plus a generous annual bonus of $40,000, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and a 401k (up to 4% match).
