Part Time Weekend Admission Registered Nurse (RN)
Anticipated Territory:
Marin County
Schedule:
Every Saturday and Sunday Full Days. Would consider Every Saturday and Sunday plus 3 week days.
What you'll do:
Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:
- Conduct Start of Care (SOC) Admissions - OASIS
- Conduct follow ups, evaluation visits as needed in designated territory
- Make home visits to patients in designated territory
- Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
- Monitor clients' conditions, reporting changes to Clinical team
- Collaborate with physicians and agency staff to deliver the best care possible
- Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.

To be a success:
- Current and unrestricted CA RN license
- Current BLS/CPR Certification
- Valid CA Driver's License
- Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
- High level of clinical knowledge in home health care is highly preferred
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals is highly desired
- Previous experience with IV Medication infusions highly preferred
- Proficiencies initiating intravenous (IV) therapy is a plus
