* Coordinate and manage the
** MDS/RAI process
** in accordance with federal, state, and CMS regulations
* Complete accurate and timely
** MDS assessments, care plans, and supporting documentation
*** Collaborate with the
** interdisciplinary team (IDT)
** to develop individualized, resident-centered care plans
* Monitor and improve
** quality measures, compliance standards, and reimbursement accuracy
*** Provide education and guidance to nursing staff regarding MDS processes and regulatory updates
* Assist with audits, reporting, quality improvement initiatives, and survey readiness activities
* Partner with facility leadership to support clinical excellence and operational success## ## Qualifications
* Current and active
** California RN or LVN license**
* ** 1+ year of MDS/RAI experience
** in a skilled nursing or post-acute setting preferred
* Strong understanding of
** PDPM, CMS reimbursement systems, care planning, and regulatory compliance
*** Excellent organizational, analytical, and documentation skills
* Ability to work collaboratively with clinical, administrative, and leadership teams
