Part Time Weekend Admission Registered Nurse (RN)

Anticipated Territory:
Marin County.

Schedule:

Every Saturday and Sunday full days; optional additional weekdays.

• Conduct Start of Care (SOC) admissions using OASIS.
• Conduct follow‑up and evaluation visits as needed in designated territory.
• Make home visits to patients in the designated territory.
• Perform assigned duties, including medication reconciliation, wound care, treatments, and procedures.
• Monitor clients’ conditions and report changes to the clinical team.
• Collaborate with physicians and agency staff to deliver the best care possible.
• Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols.
• Accurately document observations, interventions, and evaluations related to patient care management and services provided.
• Work with dedicated concierge resources that assist with time‑consuming administrative tasks such as charting and follow‑up calls.

• Current and unrestricted California RN license.
• Current BLS/CPR certification.
• Valid California driver’s license.
• Experience conducting assessments and SOC admissions using OASIS.
• High level of clinical knowledge in home health care.
• Experience with high‑acuity patients.
• Experience working with an interdisciplinary team.
• Familiarity with case management fundamentals.
• Previous experience with IV medication infusions.
• Proficiency in initiating intravenous (IV) therapy.
• Self‑directed, empathetic, and critical thinker.
• Outstanding interpersonal and leadership skills.
• Organized and detail oriented.
