The Licensed Vocational Nurse (LVN) – Infectious Diseases is a patient-facing position within the Infectious Diseases team. This role provides both direct clinical nursing care and administrative support functions for patients with infectious diseases and other complex medical or psychosocial needs, including people living with HIV/AIDS, tuberculosis, Hepatitis C, sexually transmitted diseases, and patients receiving PrEP or PEP-related services.

Working under established protocols and in collaboration with providers, RNs, behavioral health, dental, front office, pharmacies, public health, and community partners, the LVN supports clinic flow, performs assigned LVN and MA duties, coordinates follow-up care, facilitates specialty clinic activities, conducts outreach and tracking, and reinforces patient education to improve access, continuity, and health outcomes.

Responsibilities
  • Perform care within the scope of licensure, training, clinic protocol, and provider or RN oversight as applicable.
  • Room patients, obtain and document medical histories, measure and record vital signs, and prepare patients for examination or treatment.
  • Prepare exam and treatment rooms; drape and position patients; set up instruments and equipment; and assist providers during examinations and procedures.
  • Perform clinical procedures and treatments as authorized and competent to perform, including phlebotomy, catheterization, wound care, EKGs, specimen collection, oxygen or nebulizer administration, injections, immunizations, and routine laboratory testing.
  • Clean, disinfect, and maintain exam rooms, equipment, and instruments in accordance with infection prevention standards, OSHA requirements, and clinic policy.
  • Maintain adequate inventory of medical and clinic supplies and communicate replenishment needs.
  • Support efficient clinic operations by scheduling diagnostic tests and follow-up appointments, reviewing schedules, verifying patient readiness for visits, and helping maintain patient flow.
  • Verify or support verification of eligibility, demographics, required documentation, and completion of patient forms in coordination with front office staff.
  • Enter, update, and maintain accurate and timely documentation in the electronic health record and other required tracking systems.
  • Manage and update tracking lists and outreach tools for patients with HIV/AIDS, TB, Hepatitis C, STDs, and other assigned infectious disease or complex care conditions.
  • Facilitate or support drop-in TB and STD clinics under RN and provider oversight.
  • Assist with follow-up on laboratory results, patient phone calls, chart reviews, in-basket messages, and care coordination tasks within established turnaround times.
  • Respond to patient questions regarding lab results, medications, symptoms, and disease processes within scope, and elevate to the RN or provider when clinical judgment or further evaluation is needed.
  • Reinforce patient education provided by the care team, including education related to PrEP, HIV, Hepatitis C, active or latent TB, STDs, asthma, diabetes, glucose monitoring, medications, wound care, and reportable symptoms; provide approved handouts and document education provided.
  • Coordinate outreach and engagement activities to support continuity of care and reduce gaps in treatment for specialty clinic patients.
  • Assist with medication refill workflows per protocol and coordinate with pharmacies and care team members to support timely access to prescribed and specialty medications.
  • Coordinate the ordering, delivery, storage, and administration of patient specific specialty medications.
  • Initiate or support referrals to internal services, public health programs, home health, specialty care, and community-based resources as directed.
  • Complete required reporting, documentation, and case coordination activities for infectious disease patients, including communication with public health and participation in case conferences as assigned.
  • Perform Medical Assistant duties as needed such as medication/vaccine administration, ear lavage, diabetic foot exams, history taking, point of care testing.
  • Collect specimens including blood, urine, rectal swabs, oral swabs, and vaginal swabs.
  • Serve as an infectious disease clinical resource to staff regarding workflow, patient safety, and escalation of patient concerns.
  • Maintain patient confidentiality and safeguard protected health information in accordance with HIPAA and organizational policy.
