HHS) we treat every patient with empathy and compassion, providing care in the patient’s home.

• Providing hands‑on patient care within RN license scope: PICC lines, wound vacs, feeding pumps, NG/ G/J tubes, sutures, drains, Foley catheters, trach care, blood draws, ostomy management, injections, IV administration, compression wraps, and other skilled nursing procedures.
• Delivering direct patient care as defined in the State Nurse Practice Act.
• Implementing a plan of care initiated and directed by the Case Management RN.
• Documenting care accurately and timely within 24–48 hours.
• Assessing and educating patients and families on diagnoses and care plans.
• Assisting the RN with skilled procedures and self‑care education.
• Coordinating home‑health services and reporting needs to other disciplines.
• Using equipment and supplies efficiently.
• Participating in personal and professional growth activities.
• Performing other duties assigned by the Case Management RN.
• Communicating patient information effectively to the Case Manager and providers.
• Attending weekly case conferences as required.
• Maintaining patient privacy and confidentiality of medical records.
• Upholding professionalism and seeking opportunities to enhance clinical skills.
• Meeting productivity standards set by the Case Management RN.
• Reviewing Start‑of‑Care assessments and plans with the Case Manager.
• Participating in on‑call duties per weekend rotation.

• One year of recent nursing experience (internal candidates and new graduates may be considered).
• Preferred:
  One year of home‑health care experience.
• Experience with high‑acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Bachelor’s degree from an accredited nursing school.
• Current California Registered Nurse (RN) license.
• Current Basic Life Support (BLS) card.
• Valid California driver’s license with acceptable driving record.
• All licenses and certifications must be current at hire and maintained throughout employment.
• Self‑directed, critical thinker with outstanding interpersonal skills, organization, computer literacy, observation, verbal and written communication, problem‑solving, basic math, and competency checklist skills.
• Must complete annual compliance training and certifications.
