Case Manager-Medical​/Surgical Unit

Washington County Regional Medical Center (WCRMC) has proudly provided patient-centered care to our community for over 60 years. Our hospital is a licensed acute care facility with 56 beds and a 60-bed Extended Care Facility.

We are seeking a compassionate and driven Case Manager to help guide patients through their healthcare journey and ensure they receive coordinated, high-quality care.

• Assess patients’ medical, psychosocial, and financial needs
• Develop and manage individualized care plans
• Coordinate care across providers for seamless service
• Educate patients and families on treatment options and resources
• Assist with pre-authorizations and appeals
• Monitor progress and adjust care plans as needed
• Maintain accurate documentation and case tracking
• Identify barriers and connect patients with support services
• Advocate for patients during and after their stay

Qualifications

• Graduate of an accredited Nursing School
• Current RN or LPN License (GA)
• Current BLS Certification
• Minimum 1 year Case Management experience
• Strong communication and organizational skills
• Ability to manage complex situations and prioritize effectively
• Team-focused with a passion for patient-centered care
