Home Health Occupational Therapy Assistant (COTA)

Eden Health -  - Responsibilities:
Implement treatment plans ordered by the physician as indicated by the plan of care to all patients on caseload;
Review caseload with supervising OT on a regular basis as established by best practice and/or State guidelines;
Educates patients, their families/caregivers, and other staff as appropriate, in appropriate occupational therapy modalities;
Delivers occupational therapy modalities appropriate to the patient’s clinical status and needs, and in accordance with the standard of care for patient’s age;
Provides clear, concise documentation in medical record as it relates to reimbursement guidelines and Agency policy and procedure
