Our RN Case Managers work to ensures the patient has an advocate for needed services and any needed education related to inpatient hospitalization, outpatient hospitalization, transfer, placement or discharge. They are responsible for ensuring that hospitalized patients are actively receiving the prescribed treatments, have appropriate discharge plans, are transitioned to the appropriate level of care, and have an advocate for services along the continuum of care.
The Quality Department oversees, quality, patient experience infection prevention, risk and case management. The team is a fantastic dynamic group who all have different duties but work collaboratively and have a great working relationship with providers, hosptial departments and administrators.
Education- Degree in Nursing from an accredited nursing program
- Current Idaho Registered Nurse license
- Two years’ experience in a clinical inpatient setting
- Hospital Case Management or Social Work preferred
