What You Will Do:

  • Enter, coordinate, download, maintain, and verify EQRS patient data for CMS. Meet all established deadlines. Correct data when necessary to assure 100% accuracy for paid for performance standards and quality improvement projects.
  • Directly involved with the day to day operations of Sandusky Dialysis. Performs clerical duties for Sandusky Dialysis, Bellevue Dialysis, Inpatient Dialysis and director including answering phones, record keeping, filing, scanning, faxing, managing office supplies, taking messages and ensuring they get to the appropriate person as well as making entries into patient medical records when appropriate. Maintains communication with the 3 dialysis depts, other hospital departments, nephrology and vascular offices, community partners, billing company and other outside agencies.
  • Build an electronic medical record (EMR) chart for all new admissions. Obtain insurance approval and preauthorization. Set up and establish lab services. Obtain medical records from Firelands Health Meditech system. Obtain records from referring agency/hospital/nephrologist. Assemble admission packets.
  • Update and maintain medical records daily on all patients including changes to address or insurance, new procedures, hospitalizations, new diagnoses, and treatment history on all patients. Maintain a small paper chart on all patients. Thin and store paper charts according to policy. Handle medical records requests from hospitals, doctor offices, agencies and insurance companies.
  • Maintain accurate patient data with lab system. Prepare requisitions. Set up tubes and print labels for all lab draws. Assist in packaging and shipping out specimens. Maintain and restock lab supplies.
  • Coordinate with patients traveling to and from Firelands Dialysis: choosing a location, sending and receiving medical records, insurance approval, clinic location and time.
  • Assist social worker with paperwork required by Medicare and insurance companies. Maintain up-to-date and accurate copy of all insurance cards. Scans cards into EMR. Completes the MSPQ insurance quarterly reports. Offers KDQOL patient survey annually to every patient and enters patient data into the KDQOL website. Documents in EMR when survey was offered, completed or refused.
  • Create and generate reports for billing purposes, nephrologist office, IPRO (renal network), payroll, productivity, and unit management. Assemble reports and prepares handouts. Types patient education materials, meeting minutes, correspondence, and reports for the director.
  • Serve as payroll time editor for all 3 dialysis departments.
  • Coordinates IT and maintenance repair requests.
  • Prepares and maintains confidential employee personnel files.
