Health and Wellness Director

The Commons of Providence, a CHI Living Community, has an exciting leadership opportunity for a Health and Wellness Director who is committed to serving and growing our Catholic ministry. The Health and Wellness Director will lead a team of dedicated professionals committed to our values of Compassion, Inclusion, Integrity, Excellence, and Collaboration

Health and Wellness Directoris responsible for the overall leadership and daily operations of the Assisted Living and Memory Care community. This position provides oversight of resident care, resident experience, regulatory compliance and employee oversight. Responsibilities include the development, implementation, and evaluation of policies, procedures, and standards of practice in collaboration with the CHI Living Communities.

Working collaboratively with the Executive Director, interdisciplinary team, residents, families, physicians, and community partners, the Health and Wellness Director promotes resident-centered care that enhances the health, safety, dignity, and quality of life of each resident. This position is responsible for ensuring compliance with Ohio Administrative Code, CHI Living Communities policies and procedures, and all applicable federal, state, and local regulations while fostering a culture of excellence, accountability, education, and continuous quality improvement.

• Bachelor’s Degree from an accredited School of Nursing
• Registered Nurse with a current unencumbered license in the state of employment
• Current Ohio Assisted Living Certification or obtain it within 1 year of employment
• Current CPR certification
• Certified as a Gerontology Nurse (preferred)
• Five years of experience related to geriatric leadership (preferred)
