The Certified Nurse Practitioner in the Nursing Home serves as a key member of the Institutional Interdisciplinary Care Team, providing care management, clinical assessments, and education to members, families, and staff.
Responsibilities- Triaging care needs using HRAT scores
- Conducting ongoing assessments
- Facilitating access to services
- Evaluating patients post-discharge
- Advocating for self-management
This full‐time role involves visiting 1–2 nursing homes daily, ensuring timely diagnosis and intervention with a Monday–Friday schedule, 8 am–4:30 pm, offering a great work/life balance.
Requirements- Valid Ohio Nurse Practitioner license
- At least 3 years of healthcare experience, preferably in HMO/PPO settings
