The Firelands Health Cancer Center is seeking an experienced RN, Oncology Patient Navigator to coordinate services and guide patients and families through the cancer care continuum.
You will work with the multidisciplinary team to assess patient needs, set goals, coordinate care pathways, and help patients overcome barriers to timely diagnosis and treatment while maintaining accurate documentation and education materials.
