We are hiring a full-time Physical Therapist to join our dynamic care team at Cape Heritage Rehabilitation & Health Care Center in Sandwich!

As a Physical Therapist, you will provide patient/resident assessment, care planning, direct treatment, and consultation to a general rehab caseload, while participating in the clinical team process. You will also teach and train caregiver staff and supervise other therapists, assistants, aides, and students as required.

Education & Experience:
  • Bachelor of Science Degree, Master of Science, or Doctoral degree in Physical Therapy.
  • Appropriate licensing requirements, or in the process of obtaining one.
Duties & Responsibilities:
  • Screen patients/residents.
  • Evaluate patients/residents and develop appropriate care plans.
  • Perform administrative duties such as assigning and overseeing resident care activities, medical record oversight, ensuring physician orders are followed, and ordering supplies.
  • Provide direct resident treatment under a physician’s order, including individual, supervised, and group sessions.
  • Contribute to team meetings and decision-making processes.
  • Consult with and train patients, residents, and caregivers.
  • Monitor efficiencies collaboratively with supervisors.
  • Document activities promptly using the company’s clinical documentation system.
  • Apply Medicare and other reimbursement regulations consistently.
  • Communicate effectively with patients, residents, team members, and supervisors.
  • Offer clinical consultation to the facility.
  • Participate in clinical program development and implementation.
  • Train and orient staff, students, and volunteers.
  • Participate in staff interviews, census building, marketing activities, and quality improvement initiatives.
  • Ensure service and operational excellence to deliver quality outcomes.
  • Protect residents’ health information in accordance with privacy policies.
  • Show courtesy, compassion, and respect to all individuals.

  • Ability to work effectively in multifaceted systems and interact with all staff levels and customers.
  • Strong communication, organizational, and analytical skills.
  • Capability to perform documentation, equipment manipulation, and direct patient care.
  • High attention to detail and concentration.
  • Ability to handle periodic stress related to census and staffing changes.
  • Effective time management and prioritization skills.
  • Ability to work with diverse personalities, ages, and cultures.
  • Proficiency in English language comprehension and articulation.
  • Ability to stand and walk for extended periods.
