Responsibilities- Provide admission, case management, and follow‑up skilled nursing visits for home health patients.
- Administer ongoing care and case management, providing necessary follow‑up as directed by the Clinical Manager.
- Confer with physician to develop the initial plan of treatment based on orders and patient assessment; provide hands‑on care, manage and evaluate the plan, teach the patient, and revise the plan in consultation with the physician as required by policy.
- Coordinate appropriate care with various healthcare personnel such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
- Report patient care, condition, and progress to the physician and Clinical Manager continuously.
- Implement the patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs.
- Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing written care instructions that reflect the current plan of care and monitoring appropriate documentation.
- Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
- Prepare appropriate medical documentation on all patients, including case conferences, contacts, medication order changes, re‑certifications, progress updates, and care plan changes; prepare visit/shift reports and updates, and confer with other health care disciplines.
- Diploma, Associate or Bachelor Degree in Nursing.
- Minimum of one year nursing experience preferred.
- Strong experience in medical‑surgical, ICU, ER, or acute care.
- Home Health experience is a plus.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver’s license, auto insurance and reliable transportation.
- $54.00 – $75.00 per visit/unit.
- $84,900 – $116,800 per year base pay.
