Position

The Hospice Clinical Director is responsible for coordinating and managing patient care. They assist the Director of Nursing in maintaining the quality of patient care in accordance with the Agency’s objectives and policies, through planning, coordination, implementation and evaluation of hospice services. This market services the Sandy Springs region in Georgia.

Responsibilities
  • Coordinate and oversee direct and indirect patient services provided by clinical personnel.
  • Provide supervision and guidance to clinical staff to ensure quality of services and maintenance of standards.
  • Perform chart audits.
  • Gather data for QAPI and participate in QAPI activities.
  • Appraise clinical staff performance during probationary periods and annually; provide teaching and counseling to enhance service delivery and professional development of nursing and health aide staff.
  • Identify and evaluate patient needs and appropriateness of home care by reviewing the initial intake screening form before assigning direct care staff. Assist with the orientation of new staff. See patients in the home setting as needed. Maintain adequate and appropriate medical supplies.
  • Assist with orientation of new personnel as well as in-services for current staff.
  • Ensure Medicare, state and ACHC regulations are followed by all clinical staff.
  • Assist the Director of Nursing with responsibilities as requested.
  • Commit to self-development by pursuing formal education, attending workshops and conferences, participating in professional meetings, and/or conducting individual research and reading.
Job Conditions
  • Position may be stressful.
  • Primarily a desk job with minimal and occasional lifting of medical supplies and charts.
  • Occasionally bend, stoop, reach and move patients; lift up to 250 pounds and carry up to 30 pounds.
  • Travel to and from patients’ homes when supervision of care is scheduled.
  • The ability to access patients’ homes that may not be wheelchair accessible is required.
  • Hearing, eyesight and dexterity must be sufficient to perform physical assessments and patient care.
  • Use of computer, multi-line telephone, calculator, copier, and other office and medical equipment.
Company Information
  • Has access to patient medical records, patient financial accounts, and department staff records.
Qualifications
  • Must be a graduate of an accredited School of Nursing.
  • Must be a Registered Nurse licensed in the state of GA.
  • The Clinical Supervisor must possess at least one of the following:
  • A Bachelor of Science in Nursing with two (2) years combined public health nursing and progressive professional responsibilities in public health nursing OR three (3) years combined public nursing and progressive professional responsibilities in public health nursing.
  • Must have a minimum of 2 years nursing experience in home hospice or skilled home care (hospice preferred).
  • Ability to market and deal tactfully with customers and the community.
  • Knowledge of business management, governmental regulations, and ACHC standards.
  • Ability to communicate well verbally and in writing; demonstrate self-confidence and a positive attitude toward self and others, with commitment to goal achievement.

Starting at $90,000 per year
