Case Manager; Field- Office

Job Description

This is a hybrid role that includes fieldwork and remote responsibilities and requires a minimum of 2 in-office days per week for collaboration and case coordination.

• Manage and oversee assigned members’ care plans
• Conduct monthly phone contacts and quarterly in-person visits
• Advocate for members’ clinical and service needs
• Coordinate communication between members and providers
• Monitor compliance with approved service plans
• Recommend care plan modifications as needed
• Maintain thorough and accurate documentation
• Participate in collaborative weekly team meetings

• Bachelor’s Degree (Social Work, Human Services, or related field preferred)
• 2+ years of experience in case management, healthcare, or human services
• Strong organizational skills and ability to manage multiple priorities
• Comfortable with field work, documentation, and a hybrid work environment
• Valid driver’s license and reliable transportation
