Clinical Manager; RN - Home Health

We are seeking an experienced Clinical Manager (RN) Full-Time to lead our home health team. This role oversees clinical operations, ensures compliance with Medicare Conditions of Participation, and supports staff in delivering high-quality, patient-centered care in the home setting.

• Supervise and support nurses, therapists, and aides
• Ensure care is delivered per physician orders and individualized care plans
• Review documentation for accuracy, timeliness, and compliance (OASIS experience preferred)
• Maintain compliance with Medicare, Medicaid, and state regulations
• Lead case conferences and collaborate with the interdisciplinary team
• Participate in audits, QA/PI initiatives, and survey readiness
• Assist with hiring, onboarding, and staff development
• Monitor productivity, scheduling, and clinical performance
• Address patient and staff concerns promptly

• Active RN license in Utah
• Minimum 1-2 Years Home Health Experience Required
• 1+ year leadership/supervisory experience preferred
• Strong knowledge of Medicare CoPs and home health regulations
• OASIS and EMR Experience Preferred
• Excellent leadership, organization, and communication skills
