Job SummaryThe Registered Nurse (RN) is a core member of the interdisciplinary patient care team. RNs empower patients, lead clinic huddles, and apply evidence‑based nursing practices to deliver an exceptional patient experience that exceeds industry standards.
Job Responsibilities- Provide evidence‑based nursing care and patient education.
- Manage care coordination by connecting patients with specialists and following up to ensure comprehensive care.
- Utilize population management tools to optimize patient care based on industry recommendations.
- Perform phlebotomy services, in‑house laboratory testing, and collaborate with providers to send specialty labs.
- Screens for vaccine‑preventable diseases and administers primary care and travel immunizations.
- Manage RN schedule, ensure appointment appropriateness, and contact patients when needed.
- Comply with all policies, procedures, and protocols, including standing orders.
- Provide triage services to walk‑ins and over the phone, assess patient needs, and refer to providers as appropriate.
- Assist in on‑site care at client‑sponsored events such as flu clinics and biometric screenings.
- Participate in quality improvement programs to enhance efficiency and align with Crossover’s policies.
- Demonstrate proficiency in healthcare analytic tools, systems, and analysis.
- Participate in implementation of new health technologies, products, services, and programs to establish Crossover Medical Group as a thought leader for employer‑based health care and lifestyle medicine programs.
- Manage population health programs and clinic outreach initiatives.
- Serve as a resource to other nurses and the care team, and participate in training and onboarding of new nurses.
- Assist in performing Quality and Compliance checks required daily, weekly, and monthly.
- Contribute to incident reporting and compliance reporting.
- Perform other duties as assigned.
- Graduate from an accredited school of nursing.
- Minimum 3 years of clinical experience.
- Current Registered Nurse license in the state where practicing.
- Current BLS (Basic Life Support) certification.
- Bachelor of Science Degree in Nursing (BSN).
- Outpatient and urgent care experience.
- Knowledge of Apple, Google, Microsoft, or HP technology.
- Passion for delivering a best‑in‑class patient care experience within an integrated patient‑centered primary care model.
- Creative, flexible, and easily adapts to changing priorities.
- Energetic, takes initiative, and works autonomously and as part of a team.
- Embraces new ways of delivering healthcare.
