Job Summary

Matrix Home Care is seeking experienced and compassionate Registered Nurses (RNs) to provide Start of Care and skilled home health visits throughout the Seminole and Lake County area.

This is a great opportunity for nurses who enjoy working independently, providing one‑on‑one patient care, and making a meaningful impact in the home setting. The ideal candidate is reliable, professional, patient‑focused, and comfortable completing skilled nursing visits while communicating effectively with patients, families, physicians, and the clinical team.

Responsibilities

• Provide skilled nursing care to patients in their homes
• Complete Start of Care visits and ongoing skilled nursing visits
• Conduct patient assessments and document findings accurately
• Follow physician orders and individualized care plans
• Provide patient and caregiver education
• Perform wound care and other skilled nursing treatments as ordered
• Monitor patient condition and report changes to the clinical team
• Provide medication teaching and disease process education
• Maintain timely and accurate documentation
• Promote patient safety, dignity, and quality of care
• Communicate professionally with patients, families, office staff, and clinical leadership

• Active Florida Registered Nurse (RN) license
• Minimum 1 year of nursing experience preferred
• Home health experience preferred
• Start of Care experience preferred
• OASIS experience is a plus
• Wound care experience preferred
• Strong assessment, communication, and documentation skills
• Reliable transportation
• Current CPR/BLS certification
• Ability to work independently in the home setting
• Must be able to complete all onboarding requirements

Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
