Job Overview

The RN Transplant Coordinator will work with post‑transplant patients providing education and coordination of care after their transplant. This team has completed 954 kidney transplants since the start of the program in 1989.

Responsibilities
  • Assume a case‑manager role ensuring that potential transplant recipients and donors receive timely and efficient transplant education, work‑up, updates, and activation for transplantation when directed.
  • Maintain a designated caseload of patients during the pre‑and post‑recipient and pre‑and post‑donor transplant process.
  • Coordinate referrals, respond to roadblocks within the transplant work‑up process, and provide patient, family, and staff educational programming.
  • Ensure policies, procedures, and regulatory requirements are met so the transplant program remains compliant with governing agencies.
  • Possibly provide outreach services to regional dialysis and nephrology programs.
Schedule & Hours
  • Work shift:

    8 hours – day shifts (United States).
  • Scheduled weekly hours: 40.
  • Clinic hours:
    Monday through Friday 8am–5pm with on‑call expectations.
  • On‑call call is split evenly between all coordinators for days, nights, weekends, and holidays.
Compensation & Benefits
  • Salary range: $34.00–$48.00 per hour.
  • Sign‑on bonus: $10,000 available for qualified candidates.

Qualifications
  • Graduate from an accredited nursing program (e.g., AACN, ACEN, NLNCNEA).
  • Bachelor’s degree in nursing required, or in the process of acquiring with an estimated completion date within five years.
  • Currently holds an unencumbered RN license with the State Board of Nursing where practice occurs and/or possesses multistate licensure if in a Nurse Licensure Compact state.
  • Minnesota RN license required within six months of starting the position.
  • Obtains and maintains required department‑specific competencies and certifications.
