  • Provides patient care based on wound, ostomy and continence nursing guidelines, and applicable standards of care.
  • Serves as a liaison for wound, ostomy and continence care concerns. Provides education to team members and participates in assigned committees.
  • Assesses the physical, psychological, and social needs of assigned patients, and initiates data collection and assessment on assigned patients.
  • Contributes to development of the plan of care for each assigned patient, using appropriate resources in collaboration with interdisciplinary care team.
  • Initiates and implements nursing interventions, medical orders, and other related therapies.
  • Collaborates with healthcare team to educate patient and support system.
  • Analyzes, modifies, and implements strategies to progress the patient through their plan of care.
  • Documents a patient record that is reflective of all patient care provided and the patient's response to the care given in accordance with professional standards.
  • Monitors equipment and performs routine and complex treatments including ostomy care,
  • wound care with draining of wounds and fistulas, percutaneous tubes and fecal and urinary incontinence care.
  • Complies with mandatory reporting, legal consent, transfer protocols, and other requirements that preserve patient rights and ensure appropriateness of care.
  • Provides a clean, safe, and secure environment for patients and others by following safety policies and procedures.
  • May handle biomedical, pharmaceutical, hazardous and universal wastes per regulatory and accreditation requirements and standards. Understands and if assigned,
  • demonstrates the ability to safety handle hazardous drugs which may include receipt, storage, compound, repackaging, dispensing, transporting and/or disposing of hazardous drugs.
