GT Independence is seeking a dedicated Supports Broker (Case Manager) to coordinate daily Medicaid Waiver services and promote participant growth and independence. You will train and supervise staff, build relationships with providers and families, and help participants access community resources and non-Medicaid supports.
Travel is required as part of the role, with focus on coordinating plans (IEP/ISP) and advocating for participants within the Employer of Record model.
