Description What You’ll Do

As a Targeted Case Manager, you will work directly with children and their caregivers to coordinate behavioral health and community based services. You will play a key role in helping families identify needs, connect with resources, and build a strong support network.

Responsibilities include:

  • Conduct comprehensive assessments to identify strengths, needs, and service goals
  • Develop and maintain individualized service plans with families
  • Coordinate care among providers, schools, medical professionals, and community partners
  • Advocate for children and families to ensure access to appropriate services and supports
  • Help families navigate systems such as behavioral health services, education supports, and community programs
  • Maintain accurate and timely documentation in accordance with Maine Care standards
  • Monitor progress and adjust service plans as needed
  • Participate in team meetings and clinical supervision as required

Minimum Qualifications (Maine Care Requirements)

Candidates must meet Maine Care Targeted Case Management qualifications
, including:

  • Bachelor’s degree in social work, psychology, counseling, education, human services, or a related field

OR

  • Bachelor’s degree in another field with relevant experience working with children or families

Preferred qualifications:

  • Experience in behavioral health, child welfare, education, or community services
  • Knowledge of Maine Care services and community resources
  • Experience working with children and families facing emotional or behavioral challenges
Other Requirements
  • Valid driver’s license and reliable transportation
  • Ability to pass required background checks
  • Strong organizational and documentation skills
