** Compensation Package:
*** Compensation is piece-rate, paid per completed patient visit
* ** Per Visit:** $75 – $150 per completed visit (good-faith estimate upon hire based on type of visit)
* ** Mileage Reimbursement:** $0.72 per mile

** Job Summary
** As a
** Physical Therapist (PT)
** with Grand Care East, you will provide
** in-home physical therapy services
** according to a physician’s plan of care.
** Responsibilities**
* ** Evaluate patients and home environments
** to assess functional status, muscle strength, mobility, gait, ROM, and rehabilitation potential; contribute to the overall plan of care.
* ** Deliver and direct physical therapy treatments
** according to the physician’s plan of care, adjusting based on patient response and communicating changes to the physician.
* ** Educate patients and caregivers
** on therapeutic exercises, safe use of assistive devices, and care of equipment; provide guidance to family and other personnel involved in care.
* ** Document clinical progress**, prepare required summaries, participate in case conferences and discharge planning, and refer patients for additional home health services when needed.
* ** Supervise support staff**, including Physical Therapy Assistants and (when PT is the only skilled service) evaluate and supervise home health aide care every two weeks.
** Requirements
* ** Current
** California Physical Therapy License
*** 2+ years of physical therapy experience (
** home health preferred**)
* CPR certification
* Valid driver’s license and insured vehicle
