Float Nurse Practitioner (NP) / Physician Assistant (PA) – Mobile Primary Care | Senior Living | Home-Based Care
Join a high-growth, mobile healthcare team serving seniors across personal homes, board and cares, assisted living, and skilled nursing facilities.
We are hiring a Float Advanced Practice Provider (NP/PA) to support a large regional footprint — providing coverage for PTO, turnover, and new building launches while maintaining continuity of high-quality, relationship-based care.
This is a high-visibility role ideal for experienced clinicians who are adaptable, operationally strong, and interested in future leadership opportunities.

- Manage chronic conditions (HTN, CHF, COPD, diabetes, dementia, polypharmacy)
- Deliver dementia-informed, relationship-based care
- Collaborate with interdisciplinary partners (psychiatry, palliative care, neurology)
- Maintain strong documentation, communication, and quality standards
- Active, unencumbered Nurse Practitioner (NP) or Physician Assistant (PA) license
- 2+ years experience in geriatrics, primary care, home health, SNF, or senior living preferred
- Comfortable practicing independently in mobile/community settings
- Strong clinical judgment and panel management skills
- Adaptable, organized, and growth-oriented
- Valid driver’s license and ability to travel within region
