* ** Hourly Pay Rate:** $40.00 – $45.00 per hour
* ** Mileage Reimbursement:** $0.72 per mile

** Job Summary
** As a
** Licensed Vocational Nurse (LVN)
** with Grand Care East, you will be responsible for following the patient’s plan of care and providing treatment under the supervisor of the Registered Nurse.
** Responsibilities**
* ** Provide direct patient care
** in accordance with the state Nurse Practice Act and within LVN scope of practice.
* ** Implement the plan of care
** established by the Registered Nurse, ensuring treatments and interventions are carried out correctly.
* ** Document patient care accurately and promptly**, keeping records consistent with the established plan of care.
* ** Educate patients and families/caregivers
** about diagnoses, treatments, and care plans to support understanding and adherence.
* ** Coordinate with the care team
** by reporting patient needs and changes to the RN or nursing leadership and using equipment and supplies efficiently.
** Requirements
* ** Valid CA Licensed Vocational Nurse License
* Maintain a current CPR certification
* 2+ years of experience working in direct patient care (
** Home Health experience preferred**)
* Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
