AboutThe Role
The Person al Service Coordinator (PSC)
FSP/ICM is responsible for coordinating services and supports for children, youth, young adults, and families enrolled in the High Fidelity Wraparound (HFW) Program. HFW is an intensive, team-based care coordination model designed to support individuals with complex behavioral health needs who may be involved with multiple systems, including mental health, education, child welfare, juvenile justice, and housing services.
As a member of the Full Service Partnership (FSP) team, the PSC serves as a primary point of contact for clients and families, helping to develop individualized care plans, coordinate services, connect individuals to resources, and support long-term success within their homes, schools, and communities.
Productivity ExpectationThis position has a baseline productivity expectation of 50%, generally equivalent to an average of approximately 3.5 hours of direct client service per workday. The meeting of such requirements will be addressed in accordance with the Company policies and applicable law.
Responsibilities- Conduct intake assessments and comprehensive evaluations to identify client strengths, needs, and goals
- Develop and implement individualized care plans in partnership with clients, families, and treatment teams
- Coordinate services and facilitate access to community-based resources and supports
- Provide ongoing care coordination and monitor service effectiveness
Conduct home visits and community-based meetings to support client engagement and progress - Maintain regular contact with clients, families, providers, and community partners
- Complete accurate and timely documentation, including progress notes and billing documentation
- Develop and maintain professional working relationships with agencies and service providers
- Participate in treatment team meetings and interdisciplinary collaboration activities
- Meet assigned productivity and performance expectations
- Maintain compliance with HIPAA, ethical standards, and professional boundaries
- Provide transportation and accompany clients to appointments, services, or community activities as needed
- Participate in assigned after-hours response duties as required by the program
- Perform additional duties as assigned
- Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, Sociology, Behavioral Sciences, or a related field
- Minimum of four years of experience in behavioral health, social services, child welfare, case management, care coordination, or a related field
- Up to two years of graduate education may be substituted for experience requirements on a year-for-year basis
- Experience working with individuals and families experiencing complex behavioral health challenges
- Strong documentation, organizational, and communication skills
- Ability to work independently and make sound decisions with minimal supervision
- Intermediate computer proficiency and experience with electronic documentation systems
- Valid California driver's license, reliable transportation, and active automobile insurance
- Experience working with children, youth, and families in community-based settings
- Experience in Wraparound, Full Service Partnership (FSP), case management, or intensive care coordination programs
- Knowledge of behavioral health systems, rehabilitation principles, and community resources
- Familiarity with Medi-Cal documentation standards and behavioral health services
- Bilingual skills preferred
- Experience collaborating with schools, child welfare agencies, healthcare providers, and community organizations
