Epic Community Connect Project Manager
Duration: 08/28/2026 - 03/31/2027
Location: Milestone Travel - Santa Barbara, California
Overview:
We are seeking an experienced Epic Community Connect Project Manager / CSM to lead its first Community Connect implementation and help establish and grow its Connect offering. This person will serve as both the implementation lead and primary customer-facing resource, bringing prior Community Connect experience and the ability to hit the ground running.
The first customer is an FQHC with approximately 32 providers across eight multispecialty clinic locations.
Responsibilities:
- Lead the Community Connect implementation from contracting and kickoff through go-live.
- Serve as the primary customer-facing lead/CSM throughout the engagement.
- Develop and manage the implementation plan, timeline, milestones, risks, issues, and deliverables.
- Establish required Community Connect deliverables and readiness tools, including implementation and Orion checklists.
- Coordinate across Client, the FQHC, Epic teams, and other project stakeholders.
- Drive regular customer meetings, follow-up, decisions, and accountability.
- Support appointment conversion, data migration, archival, and other implementation work streams.
- Coordinate workflows and requirements across multiple locations and specialties, including Wisdom.
- Help client establish repeatable processes and best practices for future Community Connect customers.
- Support the growth and maturation of Client Community Connect offering.
- Travel for major milestones, including kickoff, potential GLRA, and go-live.
Required Qualifications:
- 7+ years of healthcare IT project/program management experience.
- Direct experience leading Epic Community Connect implementations from planning through go-live.
- Experience serving in a customer-facing PM, program lead, or CSM capacity for Community Connect.
- Strong understanding of the Community Connect implementation lifecycle, deliverables, readiness activities, and go-live.
- Experience managing multi-site and/or multispecialty Epic implementations.
- Experience coordinating data migration/conversion and other implementation work streams.
- Ability to establish project structure, deliverables, and processes rather than stepping into an already established program.
- Strong customer relationship, stakeholder management, and communication skills.
- Ability to work independently and hit the ground running with minimal oversight.
- Ability to travel onsite for major project milestones.
