RN Clinical Supervisor, Hospice

We are hiring an RN Clinical Supervisor for the Hospice department in our Santa Barbara branch. The eventual hire can expect to spend half their time in the office and half in the field, administering patient care or nurse training.

The annual salary range at commencement of employment is expected to be between $120,000 - $130,000; pay offered may vary depending on factors including job-related knowledge, licensure, skills, and experience.

Responsibility for the planning, coordination and delivery of quality care services to patients. The Clinical Supervisor is responsible for providing guidance and supervision to the team who provides the services pertaining to the palliative and supportive services required by patients with a terminal illness. The Clinical Supervisor is a professional member of the hospice interdisciplinary team. Must be available on premises or accessible by telecommunications during operating hours.

• Manages the daily assignment and workload of the Agency. Directs hospice staff on a daily basis to ensure appropriate assignment and use of multi-disciplinary service to all patients.
• Performs nursing care and other field staff functions, as necessary.
• Visits patients as needed to:
  Instruct hospice staff regarding a specialized skill, assist with problem cases, supervise staff, assist when the caseload requires.
• Supervises all patient care provided. This supervision includes direct and indirect monitoring of professional staff interactions with patients, families, and physicians.
• Ensures that services ordered by physicians are provided in a coordinated manner. Ensures proper utilization of levels of care: routine, continuous care, GIP and respite.
• Supervises and participates as needed in the development of a comprehensive patient plan of care in conjunction with physician's orders.
• Assists in developing policies and procedures, and analyzes the effectiveness of policies and procedures. Informs, interprets, and enforces personnel and administrative policies of the Agency to the hospice team.
• Conducts both formal and informal conferences to ensure that patient care is provided in the prescribed manner.
• Provides instruction, functions as a clinical knowledge resource when needed with staff and hospice patients.
• Orients new staff to the field. Coordinates ongoing in-service training programs.
• Participates in Agency committees as appropriate. Works with Director of Patient Care Services / Branch Manager in performance improvement programs.
• Must be able to perform fully assisted transfers, with or without transfer devices. Must not have any back lifting/moving restrictions.
• Performs concurrent chart reviews, reviews intake referrals, hospice nursing notes for quality and accuracy.
• Performs facility and home supervision visits to review charts and assess staff performance and customer satisfaction.
• Provides supervision, the application of new knowledge, and encourages staff motivation.
• Participates in developing systems for effective and efficient delivery of hospice services.
• Plans and conducts weekly interdisciplinary groups (IDG), when available.
• Formally and informally evaluates agency staff on an ongoing basis.
• Supervises the coordination of supplies, equipment, and community resources needed for care of patients.
