As a Physical Therapist in a home health setting, you'll work one-on-one with patients to restore mobility, reduce pain, and prevent falls, empowering them to live safely and independently at home.

• Conduct in-home PT evaluations and follow-up visits.
• Design and implement individualized rehabilitation and treatment plans.
• Improve patients’ strength, balance, mobility, and functional independence.
• Educate patients and caregivers on safe movement and equipment use.
• Document visits using Well Sky Kinnser EMR.
• Coordinate care with physicians, RNs, case managers, and the interdisciplinary team.

• California-licensed Physical Therapist (PT).
• CPR certification (AHA-approved).
• At least 1 year of clinical experience (Home Health experience preferred).
• Organized, compassionate, and reliable.
• Valid driver’s license, auto insurance, and reliable transportation.
• Strong communication skills and clinical decision-making.

USD $120 to $150 per visit.
