Location:

Schedule:
Full-Time, PM Shift Compensation: $37–$45 per hour DOESanta Cruz Post Acute is dedicated to delivering high-quality, compassionate skilled nursing care within a supportive and team-focused environment. We are seeking a dedicated and reliable LVN to join our PM shift nursing team and help us continue providing exceptional care to our residents.
Position Summary The Licensed Vocational Nurse (LVN) plays a key role in supporting resident care through direct nursing services, medication administration, coordination with interdisciplinary team members, and ensuring adherence to care plans and facility standards. This role requires strong clinical skills, excellent communication, and a commitment to resident well-being and safety.
Responsibilities
* Provide direct nursing care in accordance with physician orders and resident care plans
* Administer medications and treatments accurately and timely
* Monitor, document, and report changes in residents’ conditions
* Assist with admissions, transfers, and discharges as needed
* Collaborate with CNAs, RNs, physicians, therapists, and other care team members
* Maintain accurate and complete nursing documentation
* Ensure compliance with all facility policies, state regulations, and safety protocols
* Support and promote a positive, respectful, and resident-centered environment Qualifications
* Current California LVN license in good standing
* Previous skilled nursing or long-term care experience preferred
* Strong clinical assessment and critical-thinking skills
* Excellent communication and teamwork abilities
* Ability to work the PM shift consistently
