Overview

Continuum Hospice is hiring a Per Diem Compassionate Admissions / Resource Nurse (RN) to provide patient care in and around the Santa Cruz County Area. The position is Per Diem, as needed.

• Review clinical referral information and respond to requests & inquiries, collaborating with the Clinical Director on eligibility and appropriateness for hospice services.
• Provide pre‑assessment and admission services to patients referred to the hospice, conducting visits at home or in a healthcare facility as necessary.
• Communicate with the Intake Department, referring physicians, and the Hospice Medical Director to ensure appropriateness of each referral and obtain required certifications.
• Apply nursing principles and techniques in patient care, offering emotional support, education, and encouraging participation in the plan of care to meet established goals.
• Participate in on‑call duties as assigned by the Clinical Director and Program Director.

• Current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing from a program accredited by the National League for Nursing, or at least one year of professional nursing experience within the last three years.
• Hospice or home health experience preferred.
• Computer proficiency in typing and various programs, including background in EMR.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and reliable transportation.

$61.00 - $66.00 per hour (average pay range). Compensation will be determined based on relevant experience and the specific responsibilities of the position and will be confirmed after the interview process.
