• Utilize your clinical skills to complete the initial assessment of patients to determine home care needs, including SOC admissions (OASIS).
• Conduct follow‑ups and evaluation visits as needed in the designated territory.
• Make home visits to patients in Santa Cruz County within a 5‑15 mile radius.
• Perform duties such as medication reconciliation, wound care, treatments, and procedures.
• Monitor clients’ conditions and report changes to the clinical team.
• Collaborate with physicians and agency staff to deliver the best care possible.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
• Initiate and administer IV medication infusions (antibiotics, diuretics) safely and in accordance with protocols.
• Utilize concierge support for administrative tasks.
• Create flexible work schedules and determine your own hours.

• Current and unrestricted California RN license.
• Current BLS/CPR certification.
• Valid California driver’s license.
• Experience conducting assessments and SOC (OASIS) highly desired.
• High level of clinical knowledge in home health care preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case management fundamentals desired.
• Previous IV medication infusion experience highly preferred.
• Proficiency in initiating intravenous therapy is a plus.
• Self‑directed, empathetic, critical thinker with outstanding interpersonal skills, leadership, and organization.
