Continuum Hospice & Palliative Care California is seeking a Full Time Compassionate Resource Nurse (RN) to provide patient care in the Santa Cruz area.
Schedule:
Tuesday
- Saturday or Wednesday
- Sunday, 1pm - 8:30pm.
You will work with the admissions and intake teams to promote hospice care, conduct pre-assessments and admissions in homes or healthcare facilities, and coordinate with referring physicians to ensure appropriate referrals and certifications.
