Hospice Santa Cruz is seeking an Admissions Nurse to join the Admissions & Intake team. This role focuses on efficient admission of hospice patients, collaboration with clinical and intake teams, and ensuring proper documentation and regulatory compliance.
Ideal candidates have an RN license in California, 3+ years of RN experience (hospice/home health preferred), and strong communication skills. Bilingual English/Spanish is a plus; travel to patient locations may be required.
