Job Summary

We are seeking an Advanced Practice Clinician (APC) – either a Family Nurse Practitioner (FNP) or Physician Associate (PA) – to provide medical care including Medication for Addiction Treatment (MAT) for opioid and alcohol use disorders, care coordination to substance use disorder (SUD) residents, and collaborative support with Integrated Behavioral Health teams. The role involves in‑person and telehealth services under the supervision of the Medical Director and Director of Nursing.

Primary

Responsibilities
  • Provide MAT services for AUD and OUD within scope of practice, including long‑acting injectable formulations.
  • Oversee contingency management services for stimulant use disorders.
  • Work cohesively with the MAT operational team—RN, peers, program assistants—toward client recovery goals.
  • Coordinate care with other SUD programs, local hospitals, probation/parole, etc.
  • Order and review labs as clinically indicated.
  • Prescribe and monitor medications, including ordering urine drug screens and confirmatory testing when appropriate.
  • Recognize care priorities and potential crises, responding promptly.
  • Assist clients in improving and maintaining stability through advocacy support and education.
  • Adhere to required infection control and aseptic techniques.
  • Meet Medi‑Cal billing requirements through appropriate documentation on the EHR.
  • Attend relevant grant learning collaboratives, coaching calls, and webinars.
  • Complete all grant tracking and reporting requirements.
  • Communicate closely with community health providers to deliver integrated care and improve continuity.
  • Exhibit a non‑judgmental, empathetic, and supportive approach when communicating.
  • Use clear, concise written and oral communication; comply with departmental policies regarding documentation and privacy.
  • Support the after‑hours provider on‑call rotation.
  • Perform additional duties as assigned.
Required Qualifications
  • Unencumbered and unrestricted license to practice as a Family Nurse Practitioner (FNP) or Physician Associate (PA) in California.
  • Unencumbered and unrestricted DEA licensure within California.
  • At least two years of experience as an APC.
  • Experience treating substance use disorders and prescribing MAT.
  • Current BLS for Healthcare Providers certification.
  • Pass a health examination including a tuberculosis screening within 90 days prior to employment or within 7 days after employment, and annually thereafter.
  • Pass a criminal record clearance.
Additional Skills
  • Proficient computer skills, including Outlook, Excel, Teams, and EHR platforms.
  • Ability to recognize the needs and concerns of diverse groups of people.
  • Strong skills in attention to detail, prioritization, and triage.
  • Ability to take initiative and work independently at the level of a licensed provider.
  • Self‑motivated and proactive in identifying and addressing challenges and engaging with clients.
Preferred Qualifications
  • Bilingual in Spanish.
  • Experience providing withdrawal management services.
