Overview
Support enrolled geriatric participants Monday through Friday, providing comprehensive medical care across settings such as clinics, homes, nursing homes, and assisted living facilities.
Responsibilities- Evaluate, diagnose, and manage medical conditions.
- Document care in electronic records.
- Participate in interdisciplinary team planning.
- Provide after‑hours on‑call support on a rotational basis, with triaged calls primarily handled by nursing staff.
- Contribute to quality improvement initiatives, develop new medical programs, and promote a positive team environment.
- MD or DO with an active license, DEA license, and board certification or eligibility in primary care.
- Experience working with elderly or frail populations.
- Postgraduate training in primary care or geriatrics.
- Bilingual skills.

Salary range: $270,000–$310,000.
