• Doctorate in Physical Therapy or Bachelor’s in Physical Therapy with NPTE passed.

The Physical Therapist/PT is responsible for the assessment of referred patients requiring rehabilitation services. Key duties include providing direct patient care to assess medical condition, functional capabilities, limitations and restrictions; establishing and administering a treatment program with goals tailored to the patient’s capacity and tolerance under physician direction; setting realistic and achievable goals; documenting and recording patient condition; and educating patients and families on appropriate physical therapy methods.

• Develop a plan of care for each patient.
• Provide skilled physical therapy services/interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet therapy goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
