Overview
NM Nurse Practitioner role lness visits- in-home (insurance) assessments. Set your own hours. Local PRN opportunities across multiple zip codes near you.
Responsibilities- Conduct Adult/Geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Collaborate with the Primary Care Physician (PCP) on patient education and provide follow-up.
- Provide services in various venues to include home visits and visits to skilled facilities.
- Perform screenings including medical history, physical exam, medication reconciliation and education.
- Identify, evaluate, and address the concerns of the member, employee or participant.
- Order and interpret diagnostic imaging and laboratory results and provide clinically appropriate interventions, additional tests, or referrals as needed.
- Administer and order point-of-care and lab-based testing.
- Refer members to PCP, case management, disease management programs, and/or appropriate specialists as necessary.
- Maintain clear documentation according to clinical standards, guidelines, and protocols; ensure PHI privacy and OSHA compliance.
- May act as a preceptor for new providers; other duties may be assigned.
- Educational Requirements:
Bachelor of Science in Nursing preferred. Master’s Degree required or commensurate experience with NP licensure completion. - Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by AANP, AACN, or ANCC (Adult, Family Practice, Geriatrics or Adult-Gerontology Primary Care) or ACNPC-AG/ACNPC.
- Current CPR certification. Multistate licensure preferred.
- Knowledge and skills: perform comprehensive geriatric/adult physical exams; proficient with e-mail and Microsoft Office; strong written and verbal communication; professional and empathetic demeanor; ability to prioritize and meet deadlines.
- Valid state driver’s license and proof of automobile liability coverage; ability to travel to assigned locations via public transportation or other methods.

Flexible Scheduling. New grads welcome. - Local PRN opportunities; 8am-7pm availability; mileage reimbursement.
- From $90.00 per assessment.
- Earn from $1,800 up to $9,000 per month (based on average 20-100 completed visits per month and pay of $90 per assessment).
- Mileage reimbursement.
