LVN ​/ LPN – LTC​/SNF

Nursing Position in Long-Term Care or Skilled Nursing Facilities

Provide skilled nursing care to patients in long-term care or skilled nursing facilities, ensuring personalized and compassionate treatment. Support the healthcare team by managing patient needs and documenting care activities accurately.

Responsibilities:

• Administer medications and treatments following physician orders and facility protocols.
• Monitor and assess patient health status, reporting any changes promptly.
• Record and update patient information in medical records accurately.
• Collaborate with healthcare team members to develop and implement care plans.
• Ensure compliance with safety protocols and infection control standards.
• Communicate effectively with patients, families, and team members regarding care.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in long-term care or skilled nursing facilities.
• Ability to follow detailed instructions and maintain accuracy in documentation.
• Strong interpersonal and communication skills.
• Knowledge of healthcare regulations and safety standards.
• Must have completed COVID-19 vaccination or hold valid exemption documentation.
