LVN ​/ LPN – LTC​/SNF

Long-Term Care Nursing Position

Provide high-quality nursing care to residents in a Long-Term Care or Skilled Nursing Facility setting. Ensure compliance with healthcare regulations while delivering compassionate patient support.

Responsibilities:

• Administer direct patient care in accordance with physician orders and facility protocols.
• Monitor and document patient vital signs, health status, and response to treatments.
• Assist with activities of daily living, including bathing, feeding, and mobility assistance.
• Maintain a safe and clean environment, adhering to infection control standards.
• Communicate effectively with patients, families, and healthcare team members regarding patient needs and progress.
• Ensure accurate documentation of all care provided and reports any changes in patient conditions promptly.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience working in LTC or SNF environments preferred.
• Strong understanding of geriatric and chronic care needs.
• Ability to follow detailed instructions and maintain high accuracy in documentation.
• Excellent communication and interpersonal skills.
• Must comply with all vaccination and COVID-19 exemption requirements.
