LVN ​/ LPN – LTC​/SNF

LVN/LPN Position

Seek a dedicated LVN/LPN to provide quality nursing care in long-term care or skilled nursing facility settings. This position involves delivering compassionate patient support and adhering to facility protocols.

Responsibilities:

• Administer medications and treatments according to physician orders and facility policies
• Monitor patient health status and document responses to care
• Collaborate with interdisciplinary team to develop and implement care plans
• Ensure compliance with state and federal regulations, including documentation and safety standards
• Maintain patient safety and comfort by implementing proper infection control procedures
• Communicate effectively with patients, families, and team members to coordinate care

Qualifications:

• Valid LVN/LPN license in the state of practice
• Proven experience in LTC/SNF or similar healthcare environments
• Strong knowledge of clinical procedures, medication administration, and documentation
• Ability to work independently and as part of a multidisciplinary team
• Excellent communication and interpersonal skills
• Must meet facility-specific vaccination requirements, including COVID-19 protocols
