LVN ​/ LPN – LTC​/SNF

Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure the delivery of high-quality patient care while adhering to facility policies and protocols.

Responsibilities:

• Administer prescribed medications and treatments accurately.
• Monitor patients’ health status and document changes.
• Collaborate with healthcare team to develop and implement care plans.
• Maintain patient safety and adhere to infection control protocols.
• Communicate effectively with patients, families, and staff regarding care needs.
• Follow all legal requirements and facility policies during shift.

Qualifications:

• Valid LVN or LPN license registered in the state.
• Proven experience in long-term care or skilled nursing facilities.
• Demonstrate strong clinical assessment and documentation skills.
• Possess current CPR and first aid certification.
• Ability to work effectively within a team environment.
• Flexibility to work on Tuesdays and Thursdays for orientation or ongoing assignments.
