LVN ​/ LPN – LTC​/SNF

Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

Seeking a dedicated Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for local long-term care and skilled nursing facility assignments. The role involves providing direct patient care in a fast-paced healthcare environment with a focus on quality and safety.

Responsibilities:

• Administer medications and treatments as prescribed by physicians.
• Monitor patient vital signs and report changes to the healthcare team.
• Document patient care activities accurately and promptly.
• Collaborate with multidisciplinary team members to develop and implement care plans.
• Follow facility protocols and adhere to safety standards at all times.
• Maintain compliance with infection control and confidentiality policies.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Current CPR and BLS certification.
• Minimum of 1 year of experience in a LTC/SNF setting.
• Strong understanding of medication administration and patient care procedures.
• Ability to wear a photo badge and present valid  start of assignment.
• Willingness to operate within legal and facility-specific vaccination requirements.
