LVN/LPN for Long-Term Care/SNF Facility

We are seeking a dedicated LVN/LPN for a Long-Term Care/SNF facility. The ideal candidate will be responsible for providing quality nursing care to residents, ensuring compliance with healthcare regulations, and maintaining a safe environment.

Responsibilities:

• Administer medication and treatments accurately according to care plans.
• Monitor residents' health status and record vital signs.
• Collaborate with the healthcare team to develop and update care plans.
• Communicate effectively with residents, families, and staff regarding patient needs.
• Ensure infection control protocols are strictly followed.
• Document all patient interactions and care activities thoroughly.

Qualifications:

• License as an LVN or LPN in the state of practice.
• Proven experience in LTC or SNF settings preferred.
• Ability to work effectively in a fast-paced environment.
• Strong communication and interpersonal skills.
• Knowledge of state and federal healthcare regulations.
• Must present a valid  have completed COVID-19 vaccination or approved exemption.
